PRIVACY / WEBSITE FORMS

Your information.
Handled with care.

This page explains what the Campbell Creates website forms ask for and how submissions are used. It describes current site behavior and does not replace a full legal privacy notice.

What forms may collect

Depending on the form you choose, we may collect your name and email address, role or organization, an organization or project idea, service interests, a preferred timeline and way of working, and details you choose to share. The community connection form also records whether you are making a mentorship-program recommendation, exploring collaboration, or sharing a resource or general idea.

The caregiver mentorship interest form asks for a parent or guardian’s name and email, a youth age range, and optional first name and interests. The adult creative opportunity form may include an optional resume or project file. The resource suggestion form collects a public resource link, region, topic, and your reason for suggesting it.

How submissions are used

We use submitted details to understand and respond to inquiries, review resource suggestions, consider adult collaboration interests, and follow up about future mentorship information when appropriate. Submitting a form does not create a client relationship, confirm a position, or enroll a young person.

Inquiry, mentorship, opportunity, and resource submissions are stored in private Campbell Creates inboxes for review. Newsletter signups are separate: when the Loops connection is enabled, the email address and optional first name will be sent to Loops for Creative Connections updates. Each newsletter will include an unsubscribe option.

Signup details are sent from the website through a server-side connection; the Loops API key is not exposed in the page.

Files and youth information

Resume and portfolio uploads are optional and limited by the form’s file types and size. Please share only material you have permission to provide, and do not upload confidential information.

For youth mentorship, a parent or legal guardian must submit the caregiver interest form. The community connection form is for adult referrals and introductions; it does not need a young person’s identity. Please do not include a youth’s name, school, contact details, birth date, home address, health information, or other sensitive details in a recommendation. These forms gather early interest only and do not enroll a young person.

Newsletter choices

Signing up is optional. The form requests an email address and, optionally, a first name. You can unsubscribe from a newsletter using its unsubscribe link. If signup is temporarily unavailable, the form will say so and will not claim that your address was added.

External links and records

Links to LinkedIn, Spotify, and resource organizations take you to services outside Campbell Creates. Their own privacy policies and practices apply there.

Submissions are retained in the site’s private records for follow-up and business or program administration. Retention periods and request-handling procedures should be finalized before the site opens to the public. To ask about information you submitted, email jesse@campbell-creates-llc.com.

Before public launch

This notice reflects the current form fields and inbox behavior. Campbell Creates will review it against its actual vendors, retention schedule, security practices, and applicable requirements before making the site publicly accessible. Do not use the website forms to send payment details or sensitive personal information.
